Case Management

Once we have been approved to provide Case Management in writing, we make all the necessary referrals and ensure full interdisciplinary working so that rehabilitation can be progressed efficiently.

We keep in touch with the referring parties, letting them know what’s happened and what needs to happen next, providing formal Update Reports against agreed budgets regularly. We re-score on the outcome scores so all parties can monitor the progress made.

We work with the individual and their family, medical and legal team, insurer and employer.

All our Case Managers are Occupational Therapists with considerable experience (Click HERE for more about our Case Managers) and have been selected and engaged due to evidence of their post qualification experience and skills in understanding the process of rehabilitation. All Case Managers are registered with CMSUK and some are affiliated with BABICM.

The Case Manager completes ongoing reports and optimum recommendations to address physical, psychological and vocational needs.

This is informed by:

  • The interview with the client, family and friends.
  • Observation of impact of the injuries on function.
  • Clinical information they have received from statutory services.

The Case Manager will regularly communicate with all parties involved in the rehabilitation process, ensuring the commitment of all stakeholders for a successful outcome. On discharge, we can provide a full overview of the intervention and progress made, any recommendations to be taken forward and a cost analysis of our intervention against client progress.
